OBJECTIVES: 1. To assess the feasibility and safety of Mesenchymal stromal cells (MSC) as a treatment option for recurrent high fistula-in-ano. 2. To study the efficacy of Mesenchymal Stromal Cells (MSC) in the treatment of recurrent high fistula-in-ano. METHOD: Patients with recurrent high fistula-in-ano of crypto glandular origin (n = 7) underwent injection of heterologous Mesenchymal stromal cells (MSC), 40 million units into the fistula tract and were followed up for a total of 2 months. If healing was not detected clinically at the end of 1 month, the patients underwent a repeat injection of Mesenchymal stromal cells (MSC) into the fistula tract. All patients were followed up at the end of 2 months clinically and MRI was done for patients who had clinical healing only. All patients were followed up for a duration of 4 months or till they were labeled as recurrence requiring another form of intervention. RESULTS: There were no adverse effects, reactions or worsening of primary disease in all 7 patients who underwent injections of Mesenchymal Stromal Cells (MSC) into the fistula tract. All patients were followed up for a period of 4 months and the procedure appeared to be safe, with no additional risk. At the end of 1 month fistula healing was observed in 4 out of 7 patients (57%) assessed clinically. Two patients had persistent fistula tracts and underwent a repeat sitting of MSC injection. At the end of 2 months 3 out of 7 (43%) patients had clinically healed fistula tracts and at the end of 4 months 1 out of 7 patients had clinically healed. Imaging done at 2 months following procedure revealed persistence of the fluid filled fistula tract in all the patients. CONCLUSION: The injection of heterologous Mesenchymal Stromal Cells (MSC) in the treatment of chronic non-healing crypto glandular fistula-in-ano appeared to be safe. However it did not heal the fistula tracts in this pilot study

A framework for six-sigma implementation in rubber manufacturing industry – an innovation between DMAIC cycle and quality improvement tools

    Get PDF
Quality improvement tools are commonly utilised by the industries to attain quality products and benefits like reduction in scrap cost and wastes. In order to achieve the proper implementation of quality tools, a theoretically developed framework inclusive of six-sigma DMAIC (Define, Measure, Analyse, Improve & Control) cycle is required. Also, the quality tools associated at each stage has thoroughly implemented on a case study which carries a real-time issue related to the quality products production. The issue has thoroughly resolved through the right application of quality tools like Failure Modes and effects analysis (FMEA) on the compression moulding technique utilised for the production of Rubber Valve Sleeve Liner (RVSL) under the umbrella of DMAIC cycle. The benefits achieved are 23.1% reduction in scrap rate, 63.8% reduction in Delivery on time (DOT) with savings gained almost RM26,880/month. The relationship between DMAIC cycle and quality tools for quality improvements under the platform of lean six sigma which is highly beneficial for academicians, practitioners and engineering managers working in this field with a limitation of implementation only in the rubber manufacturing industry. In future, the developed framework should be implemented in some other manufacturing industries

Enhancing Carbon Sequestration Using Organic Amendments and Agricultural Practices

    Get PDF
Carbon sequestration (CS) is an important strategy for the mitigation of climate change (CC) as well as for improving the soil fertility of agricultural soils. Carbon sequestration in crop lands and rangelands requires a certain amount of organic matter (OM) presence in the soil called soil organic matter (SOM). Organic amendments like animal and poultry manures, the incorporation of different crop residues, different types of compost, sugarcane bagasse, peat soils, different wood chips, biochar and good agricultural practices like cover crops, nutrient management, mulching, zero and no-tillage techniques, soil biota management and mulching are effectively used for this purpose. These enhance the SOM and improve the soil’s physical and chemical properties which help to sequester more C in soil which ultimately contributes towards CS and CC mitigation

Supply Chain Policy for the Worldview of Innovations

Abstract— Innovation is an important activity to ensure the organization's sustainability which strongly depends on the supply chain policy of the system. In addition to generating profits to the organization, innovation should also be an act that can warrant well-being not only to the present generation's life, but to future generations as well and for Islam, including well-being in the hereafter. This means, innovation is not just an ordinary activity, but a responsibility to be implemented. Nevertheless, the approach of innovation between the west and Islam is different. Before an innovation could take place, it is important and worthy for an innovator to comprehend the worldview of both civilizations as there are ethical boundaries that separate the objectives, ways, and direction of an innovation. This article discusses perspective of innovation from both Islamic and western worldview and its relationship with innovation in narrative methodology
